• Witness/Sign Statutory Declarations (FREE)

• Passport Applications (if the Pharmacist has personally known the Applicant for greater than 12 months) (FREE)

• Power of Attorney Forms (FREE)

• Certifying True Copies ($1.00 PER PAGE)
